The Andrews X Ray, Inc. (AXI) web site promotes trust, privacy and security. AXI has adopted this policy to communicate commitment to your privacy. This policy is applicable to the AXI website and any other website or online communications facilities owned or operated by AXI. AXI reserves the right to change this policy at any time. Using our website demonstrates your consent to AXI's collection and use of information pursuant to this policy, and continued use of this site following the posting of changes to this policy will signify your acceptance of such changes.

This policy governs the use of your name, e-mail address, payment information, and similar personal information that you may provide AXI in the course of using our website, products or services.

AXI collects personal information when you use the contact form.

AXI's e-mail announcements: AXI occasionally uses e-mail to send announcements regarding new products, services, and/or promotions, and informational newsletters that would be of interest to our customers.
Announcements via email are on an opt-in basis only. By submitting material through our contact form, you are opting in to receiving these e-mail announcements. You can choose to unsubscribe to AXI e-mail at any time by emailing sales@andrewsxray.com with the subject line: "Unsubscribe". AXI will also send replies to customer e-mail via e-mail or telephone. If you have sent a request to AXI about which we need to communicate with you, we may do so by e-mail.

Security: Although AXI carefully selects third parties, is sensitive to the privacy and security of your personal information, and intends to safeguard such information, AXI and any such third party cannot guarantee absolute security for such information.
Such information may be transmitted through or using the internet, other public telecommunications networks, servers, hardware and software ("systems") that are not owned, operated, or controlled by AXI, and AXI makes no assurances that such information will remain private when transmitted by AXI through such systems. Protected Health Information: (called “PHI”) AXI provides services to Healthcare Providers/Covered Entities that involve the use and disclosure of protected health information under services that relate to the installation, calibration and maintenance of medical imaging equipment. AXI may make any and all uses of protected health information necessary to perform its obligation as set forth in applicable installation/service contracts with the client.
Additionally AXI may disclose protected health information for the purposes authorized by this policy and only to authorized employees, subcontractors and agents of AXI.

AXI is potentially exposed to PHI in two distinct ways:
A.) As an installer and service/maintenance provider, service can be peripherally, not intentionally, exposed in the course of performing their responsibilities maintaining medical imaging equipment.
B.) AXI is potentially exposed when transporting dated x-ray films (purged) to be channeled to the reclaiming company (recycled).

AXI has a policy not to use and/or disclose the protected health information only as permitted or required by this policy or as other wise required by law.

AXI has a policy to request the minimal amount of protected health information, which will adequately meet its intended needs.

AXI has a policy to notify the appropriate parties of any disclosure or unauthorized use of the protected health information that is not required.

AXI has a policy to use reasonable efforts to maintain the security of the protected health information.
All informational content for products is the intellectual property of the respective owners.
Andrews X-Ray, Inc. And Hudson Digital Systems are legal affiliates and each reserve the right to do business under the title of the other.

AXI will require all of its employees, representative, subcontractors or agents that receive or use or have access to protected health information under this policy to agree and adhere to the same restrictions and conditions on use and/or disclosure that apply herein.
